Studio Use Form

By choosing to utilize the studio facilities of SLDP, located at 430 E. South Temple, Suite C, Salt Lake City, UT 84111, I voluntarily acknowledge and accept the inherent risks. These risks may include, but are not limited to, injury to myself, damage to my personal equipment or SLDP-owned equipment, and potential harm to any individuals with me. I understand that my use of the SLDP facilities may be unsupervised and self-directed, with no direct oversight by an SLDP representative. I willingly assume all risks associated with the unsupervised and independent use of the SLDP studio and its equipment.

This Agreement shall be governed by and construed in accordance with the laws of the State of Utah.

Upon signing this document, I agree to indemnify SLDP, LLC., its representatives, officials, employees, and assignees, holding them harmless from any loss, cost, damages, expense, or liability arising from my use of the SLDP facilities. These liabilities may include death, sickness, injury, or property damage.

I accept personal liability for any damage caused by my actions or negligence to my equipment, SLDP-owned equipment, and the facilities, including but not limited to furniture, computers, production equipment, DJ equipment, midi controllers, audio interfaces, and bathroom facilities. I commit to promptly compensating SLDP, LLC. for any such damages that occur under my supervision. I understand that I bear responsibility for anyone entering, exiting, or using the facilities under my supervision. Treat the equipment as if it was your own, gently.

I understand that food or drink of any kind is not allowed in any studio room or near any studio equipment, and that bringing food or drink into the studio will result in the loss of my ability to use the studio outside of the regular class meetings.

I commit to make sure the studio schedule runs without interruption and to respect other students time. I will arrive just before my scheduled time, and leave promptly on the hour or just before my session ends.

I commit to ensure the security of the room that I have booked and the SLDP studio as a whole. Upon arriving, if the main studio door is locked, I will make sure it is closed and locked after entering and exiting the studio. In addition, after leaving the room I have booked at the end of my session, I commit to making sure that door is fully closed and locked, and notifying an SLDP representative immediately in the event that a door is not functioning properly and/or will not lock on its own. I recognize that SLDP facilities are solely for the use of enrolled individuals. Any non-enrolled individuals must receive prior approval from SLDP before entering or using the facilities, and they must also consent to the terms within this agreement. I pledge to report any suspicious or unauthorized activity within the facility to an SLDP representative immediately. I understand that, upon booking, all guests to my studio session must be listed. Guests cannot be added or changed after a studio session is booked.

By signing this agreement, I waive any rights to institute legal proceedings to recover costs from damage to my equipment or personal injury within the SLDP studio. I certify that I am over 18, sober, and of sound mind. I have been briefed on and agree to adhere to the rules and regulations pertaining to the use of SLDP facilities and equipment. I understand that SLDP may modify these rules, with notice provided via email.

Adherence to SLDP's rules and regulations, along with this agreement's terms, are conditions for my approved use of the studio facilities. SLDP reserves the right to revoke this approval for non-compliance as necessary.

I acknowledge that I have read, understood, and accepted the terms within this Liability Acceptance & Risk Assumption Agreement.
